CASE MANAGEMENT PATHWAYS PROGRAM
COAD Pathways Care Coordination Services provide comprehensive support for expectant and new mothers. The program assists individuals with pregnancy, family planning, and baby supplies, as well
well as navigating access to food, medicine, health insurance, primary care physicians, safe housing, job training, and education. These services are delivered through a collaboration involving COAD, the Community Action Committee of Pike County, and Valley View Health Centers. All services are provided at no cost.
How Care Coordination Works

A dedicated Case Manager helps participants navigate life's challenges.

A Community Health Worker (CHW) works with participants to establish care plan activities designed to promote better health outcomes for families.

Valley View Health Centers Partnership
Valley View Health Centers operates as a Federally Qualified Health Center focusing on the connection between physical and mental health. The center accepts most insurance plans and utilizes a Sliding Fee Scale to maintain affordable access for all community members. Available services include:

Primary Care for routine check-ups and disease management.

Community Health Worker (CHW) Support
The Role of a CHW
A Community Health Worker is a frontline public health worker certified by the Ohio Board of Nursing and typically serves as a trusted member of the local community. This trust allows the CHW to bridge the gap between health and social services, with the primary goal of helping participants feel calm, informed, and empowered.
Areas of Assistance

Prenatal Support: CHWs help schedule and attend prenatal appointments, explain expectations for each stage of pregnancy, and navigate insurance, Medicaid, and healthcare systems to encourage earlier, more consistent care.

Social Determinants of Health: CHWs address non-medical factors impacting pregnancy outcomes, such as housing instability, food insecurity, transportation barriers, and financial or employment stress, by connecting families to community resources.

Emotional and Social Support: CHWs provide ongoing encouragement, regular check-ins, answers to questions, and nonjudgmental support to alleviate anxiety.

Postpartum and Parenting Support: Services extend past childbirth to include education on breastfeeding and infant care, postpartum depression screenings and referrals, and coordination of pediatric appointments and vaccinations.
