Completing Your Intake Forms Online

Getting Started

​

  • You will receive a text message containing a secure link approximately five minutes after your paperwork is generated.

​

  • Follow this link to open the Healow Sign portal.

Verifying Your Account

​

  • Choose to receive your security confirmation code via a text message or a voice call.

​

  • Input the verification code and confirm your identity by entering the patient date of birth.

​

  • Parents or legal guardians completing forms for a minor must use the child date of birth for validation.

Navigating the Paperwork

​

  • Scroll through the document to locate the light blue highlight boxes.

​

  • These light blue areas indicate the required fields you must complete.

​

  • Returning patients will find most sections automatically populated with existing information.

​

  • Only missing or required details will display a blue prompt.

Adding Signatures and Initials

​

  • Locate the dark blue boxes to identify where a signature is required.

​

  • Tap a dark blue box to reveal the signature creation tool.

​

  • Select your preferred typed initials or signature and select the Apply button.

​

  • When reviewing the Demographic Information document, apply your signature to a single line only to either accept or decline the program.

Finalizing Your Submission

​

  • Check the page indicator at the bottom of your screen to track your progress.

​

  • Continue scrolling to view all additional pages.

​

  • Wait to press the Submit button until every single page is completely filled out.

​

  • Press Submit only after reaching the final page and ensuring all light and dark blue boxes are addressed.

What is FTCA?

Federal Tort Claims Act

Under the Federally Supported Health Centers Assistance Acts of 1992 and 1995, health centers that receive funds through the Health Center Program may be deemed as employees of the Public Health Service (PHS) for purposes of Federal Tort Claims Act (FTCA) coverage for the performance of medical, surgical, dental, and related functions within the scope of employment. The Health Center FTCA Program increases the availability of funds to health centers to provide services within the Health Center Program scope of project by reducing or eliminating health centers' spending on malpractice insurance premiums for such services.

What services are covered under the FTCA Program? Deemed Health Center Program grantees are immune from medical malpractice lawsuits resulting from the performance of medical, surgical, dental, or related functions within the approved scope of project.
