Sliding Scale Fee

What is a Sliding Fee Scale?

We are a Federally Qualified Health Center (FQHC), which means we can offer a wide variety of services through the Sliding Fee Scale. The Sliding Fee Scale is in place to meet the needs of the uninsured or underinsured, providing reduced costs on all services for those who qualify.

We offer the Sliding Fee Scale to all income-eligible uninsured or underinsured patients, based on annual household income, for all services. Income guidelines and acceptable verification for the Sliding Fee Scale are listed below.

How Do I Qualify?

To qualify for the Sliding Fee Scale, you must show proof of gross annual income for all immediate family members living in your household. Gross income is all income from all sources before taxes.

Acceptable Income Verification
  • Proof of income for last 30 days

  • Social Security award letter (if applicable)

Valley View Health Centers accepts all patients, regardless of their insurance status or ability to pay.
