We invite you to participate in a brief survey to help us better understand the health needs, challenges, and strengths of our community.
As part of a community needs assessment being conducted in partnership with the Community Action Committee of Pike County, we are seeking input from patients and community members about factors that affect health and quality of life, including access to health care, transportation, housing, food, employment, and other community resources.
Your feedback is important and will help local organizations better understand community needs and inform future programs and services.
The survey takes approximately 10-15 minutes to complete. Participation is voluntary, and your responses will be kept confidential.
